Acute Liason Nurse Learning Disability

About the Role

The post holder will be a key player in the implementation of the hospitals action plan for people with a learning

disability, including attendance at the hospital Learning Disability Steering Group The post holder will advocate on

behalf of individuals with a learning disability unable to access mainstream health services. The post holder will act

as a resource for the acute hospital regarding the complex health needs of people with a learning disability. The post

holder will support the acute patient area, where necessary, in the assessment, planning, implementation and evaluation

of care plans for patients who have a learning disability and highly complex needs.
