About the Role
Main Duties and Responsibilities Give a high standard of specialist nursing care to those people referred to the
learning disability team and to assist them to attain maximumindependence and quality of life by; assessing, developing,
implementingand evaluating programmes of care and Health Action Plans in conjunction with service users / carers. Offer
support to families and service users in order for them to gainmaximum benefit from available services, resources etc.
Take responsibility for the management of a caseload. Undertake a range of assessments in conjunction with service
users,carers / families, using a person centred approach and identify andsupport Health Facilitators for people with
