Adult Social Care Case Manager, Islington
ASC Case Managers play a crucial role in coordinating care and support for residents (and their carers and families) who
contact or are referred to adult social care from the community. The team provides the central community facing access
point for adult social care enquiries and safeguarding concerns. Working as part of a multi-disciplinary team of social
workers, occupational therapists, case managers and support workers/advisers, Case Managers will manage a varied
caseload working with professionals across the team to ensure a seamless service to individuals with a range of needs.
