About the Role

Saint Michaels Hospice was delighted to launch a new person-centered advance care planning (ACP) service in July 2024

for people who have been identified as having a palliative diagnosis. The Advance Care Planning Nurse Specialist is a

new role which through working as part of a Clinical Team will increase the quality and experience of care provided for

patients with a palliative diagnosis and their families through the provision of integrated care and reducing
