About the Role

JOB DESCRIPTION

The main purpose of this role is to be an expert practitioner with highly specialised renal knowledge to develop the

delivery of supportive and palliative care to patients with advanced kidney disease. The candidate will be responsible

for providing a high quality holistic palliative care service to renal patients both in the hospital and in the

community. The post holder in collaboration with the multidisciplinary team will be directly responsible for assessing,

planning, implementing and evaluating patient care and act autonomously, using a person-centred approach within the
