Advanced Care Practitioner/ Advanced Nurse Practitioner
About the Role
The ACP or ANP will work autonomously within their professional boundaries, providing high quality clinical assessment,
diagnosis, treatment, and evaluation to patients presenting with both acute and long term health conditions. The
post-holder will manage undifferentiated and undiagnosed presentations, making complex decisions based on advanced
clinical knowledge and skills. Working as part of a supportive multidisciplinary team, the ACP or ANP will contribute to
safe, effective, and patient centred care, supporting the practices commitment to high clinical standards and continuous
service improvement. Regular debriefs and access to a supervising GP will be available throughout the working day. The
