(ACP) to join an Urgent Community Response team in Sussex.

We are looking for autonomous ACP’s with experience in community, urgent care or acute settings to support this client.

This is a brilliant opportunity to support rapid community-based care, help prevent avoidable hospital admissions, and

make a real difference to patients in their own homes.
