Advanced Clinical Practitioner - Acute Care Team

About the Role

To provideadvice, support and direction to ensure holistic patient care for acutely ill and deteriorating patients

across ELHT in conjunction with the multi disciplinary team over the 24 hour period. Work closely with the Intensive

Care Nursing and Medical Teams to facilitate early and timely admission to critical care and safe discharge from

critical care To provide the infrastructure that addresses the knowledge and skills deficits of ward teams in delivering

acute care skills to patients. Support the Implementation of national guidelines or evidence based practice to improve

patient care. Addresses clinical governance issues promptly. Provide leadership support for the Acute Care Team. The
