About the Role

Key Responsibilities Clinical Care Act as a first point of contact for patients via triage, face-to-face, telephone, or

online consultations. Independently assess, diagnose, treat, and manage patients with undifferentiated and complex

presentations. Request and interpret diagnostic tests and investigations as appropriate. Prescribe and review

medications in accordance with clinical guidelines and legal frameworks. Provide personalised, holistic care that

promotes self-management, prevention, and health improvement. Identify and manage long-term conditions where

appropriate, working closely with the practice nursing and GP teams. Refer patients to other services and agencies when
