About the Role
We have an exciting opportunity for an Advanced Clinical Practitioner (ACP) to work in our Integrated Frailty Crisis
Pathway. This role will initially sit within our community based integrated service, rotating through our Single Point
of Access and integrated Urgent Community Response/ Hospital at Home service. There will be scope to rotate through
other aspects of the Acute Frailty Service including the ED in reach team, Frailty Same Day Emergency Care and inpatient
Older Peoples Advice and Liaison. We welcome applications from both Allied Healthcare Professionals and those with a
nursing background who meet essential criteria. An Independent Prescriber qualification would be advantageous.
