About the Role

We are looking for an Advanced Clinical Pracitioner to join our forward thinking and supportive team at Cauldwell

Medical Centre (CMC) where you will be provided with an exciting opportunity to practice and further develop your

clinical skills.

Main duties at Cauldwell Medical Centre include patient triage, home and nursing home visits, chronic disease

management, acute assessments, and health promotion. The ACP supports QOF targets, reduces hospital admissions, and

mentors nursing staff. They contribute to safeguarding, clinical supervision, and continuous service improvement,
