Join Our West Kent Home Treatment Service as an Advanced Clinical Practitioner

We’re looking for an exceptional Advanced Clinical Practitioner to play a key role in delivering high‑quality,

person‑centred care to people living with frailty across West Kent. This is an exciting opportunity to use your advanced

clinical expertise, autonomous decision‑making and specialist assessment skills to support patients who are acutely
