About the Role
Care Home & Housebound Services Deliver proactive and reactive care to residents in nursing and residential homes
Support structured care home rounds and urgent reviews Contribute to anticipatory care planning, frailty management, and
end-of-life care Work closely with care home staff to provide clinical advice and guidance Multidisciplinary Working
