About the Role
Central Cheshire Integrated Care Partnership (CCICP) is looking to expand it's current Advanced Community Practitioner
workforce on a permanent basis to support Urgent Community Response and Virtual Ward.
Please be aware that a full UK driving license and access to a vehicle that can be used for work purposes are
requirements for working in a community setting.
Working as an Advanced Community Practitioner at CCICP you will be primarily conducting home visits in community for
patients that are acutely unwell and at risk of admission to hospital. You will be working autonomously to assess,
