Advanced Nurse Practitioner (ANP) / Advanced Care Practitioner (ACP)

About the Role

Working as an individual practitioner, within your scope of practice, but also a member of the wider team in delivery of

excellent levels of medical care to the practice population. You will also be an important member of the

multidisciplinary team helping to foster a positive collaborative working environment and aiding with the effective

management of the practice. You will manage appropriately triaged patient presentations, both acute and chronic, and

work with the team to help address their acute presentation as well better their future health.
