About the Role

Manage acute and chronic conditions across the lifespan Develop and evaluate individual care plans for chronic disease

management Provide advanced, evidence-based clinical care in line with NICE Request and interpret diagnostic tests

including pathology, ECGs, spirometry, and urinalysis Accurately document in accordance with legal and professional

standards Support clinical emergencies and safeguarding protocols Prescribe and review medications in line with current

guidance Promote collaborative working with community services and multidisciplinary teams Uphold high standards of

governance, infection control, and clinical accountability
