About the Role

Key responsibilities Clinical care Care co-coordination Collaborate closely with GPs, nurse practitioners, nurses,

social workers and allied health workers Communicate assessment findings and care plans clearly to the primary care team

Support care transitions following hospital discharge or emergency visits Facilitate referrals to community services and

resources as needed Documentation and Quality Accurately document assessments, interventions and outcomes in the

electronic medical record Participate in quality improvement initiative and projects Follow clinical protocols,

infection control and patient safety guidelines Patient engagement Build trusting relationships with patients, carers
