About the Role
JOB ADVERT
This is an exciting opportunity for an Advanced Nurse Practitioner to support patients with urgent on-the-day care needs
in the local community. This role will support the General Practitioners and the Multi-disciplinary Team to deliver
safe, effective and person-centred care to patients in a primary care setting. You may work in GP practices, visit
residents in care homes or attend to patients in their own homes. There is also a commitment to the Four Pillars of
Advanced Practice, allowing for the development of skills in education, mentoring, audit, leadership, and quality
