About the Role

Job description Primary Responsibilities The following are the core responsibilities of the advanced nurse practitioner.

There may be on occasion, a requirement to carry out other tasks as required. The role will require working at a number

of different practices across the PCN but travel between practices will be limited as much as possible, implement and

embed health promotion and well-being programmes. Assess, diagnose, plan, implement and evaluate treatment/interventions

and care for patients presenting with an undifferentiated diagnosis. Implement and evaluate individual specialised

treatment plans for chronic disease patients. Identify, manage and support patients at risk of developing long-term
