An exciting opportunity has arisen to join for an Advanced Nurse Practitioner to join our excellent and expanding
Frailty team at Wirral University Teaching Hospital NHS Foundation Trust.
As the population ages there are increasing numbers of older people living with frailty who are presenting to ED
departments. It is recognised that targeting this high risk group as early as possible during admission provides an
opportunity to improve quality of care and optimise patient and service outcomes for a vulnerable population.
