About the Role
To work in partnership with the Primary Health Care team (PHCT) in providing patient-focussed care responsive to health
need. To work in partnership with the PHCT in developing new services for patients. To utilise the skills / knowledge
and competencies required in order to provide first point of contact direct access for patients with undifferentiated
and undiagnosed problems seeking a primary health care service. To utilise the skills / knowledge and competencies
required in order to be responsible and accountable for assessing patients with undifferentiated and undiagnosed health
