Advanced Practitioner - Generic Therapy

About the Role

This role as an Advanced Practitioner in Generic Therapy offers a unique opportunity to support Long Term Conditions services for the Island population. You will work within a multidisciplinary team to enhance patient care, prevent hospital admissions, and facilitate early discharges, all while enjoying the beautiful Shetland environment.

Responsibilities

• Support Hospital at Home initiatives.
• Collaborate with Physiotherapy, Occupational Therapy, and Dietetics.
• Deliver care in community settings and patient homes.
• Prevent hospital admissions and enhance early discharges.
• Participate in ongoing training and CPD opportunities.

Requirements

• Relevant qualifications in therapy or healthcare.
• Experience in multidisciplinary team settings.
• Ability to work independently and as part of a team.
• Commitment to patient-centered care.
• Compliance with healthcare standards and conduct.

• Salary range of £30,353 - £33,016 per annum.

Location & Schedule

Full-time position, 37 hours per week, based in Shetland.
