Advanced Practitioner - Hospital at Home

About the Role

The Advanced Practitioners will work autonomously across Primary, Community and Acute Services during the in and out of hours period, covering seven days including public holidays. They will be responsible for the clinical decision making for the population including assessment and treatment of patients in across the Primary, Community and Acute settings.

Responsibilities

  • Provide assessment and clinical care for patients.
  • Make complex, autonomous decisions regarding care.
  • Collaborate with multidisciplinary teams and medical staff.
  • Communicate patient needs to GPs and Acute Services.
  • Utilize advanced skills and knowledge in practice.

Requirements

  • Registered Nurse/AHP/Pharmacist with valid registration.
  • Masters level education or equivalent experience.
  • Post Graduate Diploma in Advanced Clinical Practice.
  • Comprehensive knowledge of clinical guidelines.
  • Evidence of advanced practice skills and leadership.

Location & Schedule

This role will be based in Hospital at Home within Lanarkshire, with a full-time schedule of 37 hours per week.
