About the Role

We have an exciting opportunity within our Care Home Liaison Team in Knowsley for an Advanced Clinician (Registered

Nurse- mental health).

Knowsley Care Home liaison is an integrated team which supports the mental and physical health and wellbeing of care

home residents to deliver an improved level of quality of care and patient safety for residents.

Our aim is to support residents, families and carers to plan for future healthcare needs and to reduce the number of

avoidable, non-elective hospital attendances, admissions and re-admissions.
