We have an exciting opportunity within our Enhanced Health Care Home Team (Care Home Liaison) in Knowsley for an

Advanced Clinician (Registered Nurse- physical health).

Enhanced Health Care Home Team (Care Home Liaison) in Knowsley is an integrated team which supports the mental and

physical health and wellbeing of care home residents to deliver an improved level of quality of care and patient safety

for residents. Our aim is to support residents, families and carers to plan for future healthcare needs and to reduce

the number of avoidable, non-elective hospital attendances, admissions and re-admissions.
