About the Role

An exciting opportunity has arisen to join an existing nurse led service within a multidisciplinary team based at

Bournemouth Hospital but covering UHD and Dorset that cares for patients with Liver Disease, including Hepatitis B and C

and those experiencing Homelessness.

We require a Care Coordinator / Navigator to work within the project team to coordinate test results and ensuring

linkage to care and a smooth care pathway to treatment for any patient diagnosed with Hepatitis B, C or HIV through the

project. The role will also involves delivering Fibroscanning Clinics (with appropriate training) in clinic setting on
