Anticoagulant Monitoring Nurse or Pharmacy Technician - North East
About the Role
To provide efficient and safe near patient testing and monitoring of patients disease state and anticoagulation status.
To provide specialist advice and support to patients, relatives, and other health care professionals involved in the
patients care. To provide the therapeutic adjustments of oral anticoagulants within the Anticoagulant Protocol to ensure
patients are kept targeting INR. To perform dose adjustment and titration, by following the Anticoagulant Protocol. To
ensure maintenance of anticoagulation kit and equipment. Work as part of the Anticoagulant team. Promote and monitor the
appropriate and safe use of anticoagulant therapies within the service under the direction of the Contract Lead. Works
