About the Role
Providing a clinical dosing service for patients on anticoagulantscontributing to clinical care within the hospital and
community The anticoagulant practitioner will be responsible for providing specialistknowledge and care to patients.
They will undertake a range of duties toensure the effective delivery of care, providing specialist advice,structured
education and ongoing support to their anticoagulant patientsbased on individual need. The post holder will be an
experienced registered nurse responsible forthe provision of an autonomous service for patients requiringanticoagulation
therapy, but will refer for consultant nurse/haematologistadvice when secondary involvement is necessary. To dose
