About the Role
To deliver a comprehensive support service for patients diagnosed and suffering from cardiac arrhythmia’s and promoting
a seamless pathway of care for this patient group and ensuring individualised patient support, effective self-management
and education. Acting as a resource point providing educational and clinical support to health professionals within
primary & secondary care. Facilitating the implementation of NSF targets.
As an independent practitioner to assess patients in an outpatient clinic setting, assessing patients, history taking
and examining patients and formulating a treatment plan with the the support of the ANP and Consultant Cardiologist. To
