Assertive Outreach Team - Mental Health Practitioner

About the Role

An exciting opportunity has arisen within our Assertive Outreach Team, as part of our Community Mental Health Service,

based in Bridgwater. We are looking for an enthusiastic practitioner (Registered Mental Health Nurse, Social Worker or

Occupational Therapist) who is keen to support people with serious mental illness (SMI) and complex needs, across a

shared caseload.

Our team respond rapidly to the needs of adults needing mental health advice and support. The team is established,

supportive, knowledgeable and above all passionate about what they do!
