The Department for Older People has an exciting opportunity to support the development of a Band 4 Practitioner.

We are looking for Band 3 HCA who has a passion for working with Older People, to work in our Broadgreen Outpatient

Department and support the development of the newly established Frailty Hub. This post will support our MDT clinics,

whilst working with Geriatrician Consultant, Nurse Consultant and Therapist. You will be utilising extended practitioner

skills, taking a social history and supporting the completion of a comprehensive geriatric assessment. The post holder
