About the Role

Join the East London NHS Foundation Trust as a Band 4 Assistant Practitioner in our Rapid Response team. This role involves delivering high-quality, patient-centered care to housebound residents with complex health conditions.

Responsibilities

• Deliver responsive care to patients in their homes
• Support the nursing team and wider MDT
• Assist with urgent or same-day care needs
• Participate in regular supervision and mentorship
• Contribute to service needs across the integrated team

Requirements

• Excellent communication and IT skills
• Problem-solving abilities
• Experience in adult community services is desirable
• Willingness to learn and contribute
• Access to a car for service duties

Compensation

Band 4 salary, to be discussed during the interview process.

Location & Schedule

Position based in Dunstable, working 37.5 hours per week.
