About the Role

This post is to support of the Specialist Public Health Practitioners (Health Visitors and School Nurses) to deliver the

Healthy Child Programme for the 0-19 year olds.

The Healthy Family Team work in partnership with other agencies to deliver care and interventions to children, young

people and families. The team is led by the Specialist Public Health Practitioners (SPHP), with support from the skill

mix colleagues in working with 0-19 year children and young people within a defined geographic area.

Working with under age 5 will include breastfeeding support, completing mandated developmental reviews, and supporting
