Associate Advanced Nurse Practitioner - Community

About the Role

To work across care settings providing advice, education and support to patients, their carers and other healthcare

professionals, acting as an ambassador with external partners and other key stakeholders. The role will promote

preferred place of care, management of complex end of life care symptoms and managing staff, patient and carer

expectations whilst meeting the integrated care needs of the patient. Key Tasks: Develop and maintain communications

with patients, carers, other professionals and volunteers, developing specialist knowledge in palliative and end of life

care. Communicate information to distressed patients and/or bereaved family members/ people in a sensitive manner.
