About the Role
we are delighted in over a Band 4 permanent position. Based at Billingham Health Centre working within the Community
Integrated Assessment Team. This is an exciting opportunity to join the Community Integrated Assessment Team working in
line with the New NHS 10 Year Plan. Working alongside Multifidisciplinary teams, in preventing hospital admissions,
supporting safe discharges from hospital, provide a rapid response and ongoing assessment for patients either at home or
in a community setting.
Undertaking clinical tasks within competencies, supporting planning and carry out patient previews, including liaising
