About the Role
Associate Practitioners/Nurse Associates are fundamental to the delivery of integrated care, working collaboratively
with therapy, medical and nursing staff to support and facilitate the treatment of patients within their own homes.
The post holder will work as an active member of the Integrated Care Team in providing a high quality, efficient and
effective service to patients.
The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of
