About the Role

See Job Description attached The post holder will be part of a team of support workers trained in a range of skills,

delivering care and support in the community to enable patients to return home safely following discharge from hospital.

The STGH service facilitates early supported discharge and aims to support independence at home and prevent/reduce the

likelihood of increasing interventions or hospital re-admission.
