Band 2 Night Support Worker

About the Role

We are currently looking to recruit a dynamic, enthusiastic and flexible support worker who will join this multi

disciplinary team to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital

discharge, in line with the overall objectives of the service. The post holder will work as part of the District Nurse

Evening service and will provide a caring service for individuals in their own homes, under the direction of a

registered professional. They will work in partnership with patients, carers and other health and social care agencies

to support and maintain patients at home. The main aim is to provide care through-out the evening by assisting the
