About the Role

Main Duties 1. To support a comprehensive and responsive service to all inpatient areas/areas either at BCHC or other

NHS Trusts, in relation to all aspects of bed referrals management, allocation, admission and discharge planning and

facilitation. 2. To support the timely and accurate provision of data and information related to the admission and

discharge of all patients with designated inpatient areas across a 7-day service. 3. To support the recording and

management of referrals into the Single Transfer of Care Team - Central Hub related to patients who need to be

considered for D2A bed allocation. 4. To actively liaise with patients and/or their relatives ensuring they are involved
