About the Role
The purpose of the Breast Cancer Patient pathway navigator, supporting SFU is to support the Personalised Stratified
Follow-Up (PSFU) Clinical Nurse Specialists (CNSs) to deliver safe effective follow up care to patients who have had
treatment for breast cancer. The role is responsible for overseeing the transfer of patients from the Somerset Cancer
Registry database (SCR) to the Somerset Remote Monitoring System (RMS).
The role is responsible for functions within and related to the RMS including escalation of variances, generating
letters and appointments. They will support the provision of Personalised Cancer Care to patients on the PSFU pathway in
