Vanrath are recruiting for a Band 4 Community Care Support Worker on behalf of a large public sector company in

post holder will be required to provide a Community Support Worker role with adults with a physical disability, older
people and/or their Carers. This will require them to be an integral member of both the multidisciplinary team and the
community social work team. They will contribute to the assessment, care planning and arrangement of service for clients
