About the Role

An exciting opportunity to join the team and become part of the Mid Essex Dementia and Frailty Pathway this position is

a developmental role in to a band 4 , and you will be required to complete care certificate and have the relevant maths

and English within 12months to move onto the apprenticeship.

The Mid Dementia and Frailty Pathway provides a comprehensive assessment, diagnosis and treatment supporting patients of

any age with dementia as well as older adults, who have functional mental health needs with an associated age-related

frailty. You will work as part of the multi-disciplinary team supporting patients with SMI / a secondary care mental
