About the Role
In partnership with the cardiac rehabilitation nurses, you will be responsible for : Managing the cardiac rehabilitation
referral list and refer on to appropriate services as necessary Identify suitable patients for the programme at RBH
Deliver educational sessions within the programme Deliver evidence based cardiac advice, care and treatment throughout
the programme To identify patients requiring further medical review throughout the programme and discuss within cardiac
rehabilitation team To identify and act swiftly in the case of medical emergencies Principal Responsibilities: To assist
in the identification, assessment, delivery and evaluation of cardiac rehabilitation at Royal Berkshire Hospital (RBH)
