About the Role

To lead a team of clinical pathway navigators and clinical pathway administrators and be responsible for the smooth

running of the patient journey, from referral to discharge from Trust care, ensuring that coordinated and streamlined

administrative processes

revolve around the patient and their needs

This is a fixed term position for six months.

This involves developing strong working relationships and working closely with navigators, clinical pathway
