Band 5 Mental Health Nurse (Supported Discharge)

About the Role

Able to work independently in a number of environments as an autonomous practitioner whilst being supported by senior

RMN colleagues, as required. Able to work collaboratively with Band 6 RMN colleagues. Provide Supported Discharge

assessment of referred patients aswell as risk assessment to feed into hospital discharge planning. Regularly review

caseload management with their clinical supervisor. Maintain accurate records and clear documentation about the care and

treatment offered. Ensure that clinical, supportive and therapeutic interventions protect the rights of the patient and

are in accordance with evidence-based practice and Trust policies. Adhere to confidentiality principles and maintain the
