a stay in an acute hospital or may have been unwell at home and need a bit of help to avoid an admission to a big

hospital. We aim to get people back on their feet so they can return to their home by ensuring their environment is safe

and their abilities are optimized. We also accept patients who require complex discharge planning, palliative and end of
