About the Role
We are seeking an enthusiastic Physiotherapist or Occupational Therapist to join the Community Independence Service
(CIS) and support older adults in Hammersmith & Fulham to live independently.
You will join a committed, multidisciplinary team delivering high-quality patient care across the Home First/Discharge
to Assess pathway. Working alongside therapists, healthcare assistants, coordinators, Rapid Response, Reablement and
Social Care colleagues, you will support safe and timely discharges into the community. The service is co-located with
LBHF Adult Social Care in Hammersmith and works closely with Charing Cross Hospital and other local acute partners. You
