Band 6 Community Mental Health Nurse - Older Adults CMHT

An exciting opportunity has arisen for two Band 6 nurse to join our Older Adult CMHT.

Working alongside the wider MDT you will be responsible as the Care Coordinator of a caseload. This will include

completing initial assessments and coordination of care needs, planning and implementation of evidence based care to a

defined group of older adult service users and their carers within a defined catchment area/locality, and via a job

plan.
