About the Role
To assess and manage patients referred to the Ealing Rapid Response Service with complex physical, mental health and
social issues. This will include providing specialist therapy assessment and support as directed by senior clinicians to
support early discharge from hospital or avoid admission to hospital.
To deliver all elements of Occupational Therapy or Physiotherapy assessment and treatment in a community setting,
usually in the patient’s own home or care home in a timely manner.
To supervise, train and support workers and students as directed by Senior Clinicians.
