About the Role
Undertake quality comprehensive mental health and social care needs assessment, including safety assessment for service
users. Be responsible for a specific caseload of service users using the Personal Wellbeing Plan acting as Key Worker.
Establish with the service user and carer (where appropriate) an agreed plan, which includes evidence based
interventions to address assessed needs, how outcomes will be measured and agreed timescale for input. To arrange such
services as are appropriate to the plan using the Your Team Your Conversation Your Plan approach. Regularly evaluate the
plan with the service user and alter as required. Undertake specialist assessments, eg cognitive testing and Continuing
